Issued to any Arkansas resident with a disability as determined by a licensed physician or Advanced Practice Registered Nurse.
Licensed Physician's Certification Form 10-366 completed by a licensed physician or Advanced Practice Registered Nurse, certifying permanent disability. Your social security number, driver license number or state issued identification card will be required at the time of application. State and federal laws require the department to hold social security numbers and medical conditions in strict confidence. Original issuance, replacement and transfers can be done in any state revenue office.

A. The following is available for persons with a disability as determined by a licensed physician or Advanced Practice Registered Nurse: 1. A special license plate for the permanently disabled, which is issued with an expiration decal in the same manner as a regular license plate expiring annually 2. A special permanent certificate (placard) for the permanently disabled, which expires two (2) years from the last day of the month in which it is issued. A re-certification is not required to issue a new permanent certificate for the purposes of renewal. 3. A special temporary certificate (placard) which expires three (3) months from the last day of the month in which it is issued. A person certified as temporarily disabled may receive no more than two (2) temporary placards, which must have the same expiration date. Under no circumstances can a person certified with a temporary disability receive a permanent placard. B. Uses and Restrictions of Disabled Plates 1. Other than parking privileges, disabled license plates are subject to the same uses and restrictions as regular permanent license plates and are restricted to passenger cars, motorcycles and class one (1) trucks. 2. Provided however, if the applicant provides proper documentation, a larger or special purpose vehicle would be eligible to display a disabled plate. For example if the vehicle has a lift, ramp, hand controls, etc. Fees would be charged according to gross loaded weight. C. Issuance Limitations 1. Person Certified as Permanently Disabled a. One (1) special plate; b. One (1) special plate and one (1) permanent placard; or c. Two permanent placards. 2. Person Certified as Temporarily Disabled a. Two temporary placards. Both must have the same expiration. b. Temporary placard may not be renewed. D. Requirements for Permanent Placards and Disabled License Plates 1. Initial Issuance a. Licensed Physician's Certification Form 10-336 completed by a licensed physician or Advanced Practice Registered Nurse certifying permanent disability. b. Social security number, or drivers’ s license number, or state assigned identification number. E. Requirements for Temporary Placards 1. Initial Issuance a. Licensed Physician's Certification Form 10-336 completed by a licensed physician or Advanced Practice Registered Nurse certifying temporary disability. b. The applicant must provide a social security number, driver’s license number, or state assigned ID number. 2. Replacement a. A temporary placard cannot be replaced if it is expired. The replacement of an un-expired temporary placard must be given the same expiration as the placard being replaced. F. Fees and Expiration 1. Permanent placards are free and expire two (2) years from the last day of the month in which they are issued. A new placard must be issued on expiration. 2. Temporary placards are free and expire three months from the last day of the month in which they are issued. 3. License plates for the disabled are the regular fee. The expiration is same as the plate normally issued for the vehicle. G. Organizations 1. Organizations providing transportation for persons with disabilities may apply for special placards and license plates. An authorized official of the organization must complete the Licensed Physician's Certification Form 10-336 and check disability code F. 2. The organization must provide their Federal Employer ID number, which will be used in lieu of a social security, driver’s license, or state assigned ID number. H. Handicapped Motorcycle Plates 1. A special disabled plate is available for motorcycles. The same criteria for obtaining a disabled passenger car plate applies to obtaining a disabled motorcycle plate. 2.
