Outreach Event Tracking Form
This form is to track a single event in which your agency either hosted or participated in a community event where you shared information about your program. If you participated in more than one event per quarter, please complete this form separately for each event.

Goal Tracking Form
This form will keep track of your overall program goals. Please complete this form separately for each project related goals.

Technical Assistance Tracking Form
This form is to be utilized any time technical assistance is utilized. This includes meetings with the DFA-IGS, State Drug Director's Office, and Regional Prevention Representatives (RPRs).

Take Back Event Tracking Form
This form is to summarize the accomplishments of a Prescription Drug Take Back Day and is to be filled out after the event has occurred.
